Student Accident Insurance

     The school district provides supplementary accident insurance for students while at school or participating in extra-curricular activities.  This coverage is provided through K&K Insurance and is not intended to be the primary insurance coverage for students.  It is meant to be secondary coverage and therefore provides very limited coverage and benefits that do not cover all medical costs.

    One policy covers all students at school and while participating in school-related activities.  It also covers students in grades 7-12 participating in extracurricular activities and athletics.  This policy has limitations and a $250 deductible and will not cover all medical costs.  An additional policy covers most medical costs above $6,500 for students in grades 7-12 participating in KSHSAA activities and athletics.

    This insurance coverage is purchased as a benefit to our students.  USD 350 is not responsible to cover medical costs for students.  Please contact the school or district office for questions about this supplementary insurance.  Click on the link below to see the specifics of this coverage.

     What do I need to do in the case of an injury at school or during a school activity?

Parents will need to submit a claim form as soon as possible.  The claim form must be filled out by the parent and signed by a school official.  It is the parent's responsibility to submit a claim, but school personnel will be happy to submit the forms to the K&K upon request. These forms can be obtained by contacting the school office or clicking on the link below.  Claims must be filed within 30 days of an accident or injury, so it is important that parents submit a claim form as soon as possible.
